CPD Activity Application Form

To obtain CPD, all courses have to be approved by the Council unless the course is being run by a pre-approved course provider.

The osteopath or course provider should seek approval well in advance of the course. Retrospective approval will be considered but will not guarantee CPD points.

Please complete this form  with details about your CPD training course and course provider and send to us by clicking the "submit" button when form is completed. If you prefer, you may print out this form and fill in normally and fax or post this to us with necessary evidence - see our Contact Details for fax number and postal address.

After your application has been received and assessed, the applicant will be advised of the outcome, and if approved, the course will be posted on the List of Approved Courses page on this website.
